Description
Medial canthus anatomy is rendered at the inner palpebral commissure of an adult male eye, where the upper and lower eyelids converge just lateral to the nasal root. The lacrimal caruncle and plica semilunaris occupy the medial conjunctival recess, with the superior and inferior lacrimal puncta positioned on the lacrimal papillae at the free eyelid margins. A subtle lacrimal lake sits immediately lateral to the caruncle, while the medial palpebral ligaments anchor the eyelids toward the anterior lacrimal crest. Small, crowded structures. Clinically, this is the working field for evaluation of epiphora and medial eyelid malposition: punctal stenosis, canaliculitis, and nasolacrimal duct obstruction often declare themselves here with tear pooling at the medial canthus. Precise appreciation of punctal orientation and the intimate relationship of the canaliculi to the medial canthal tendon guides punctoplasty, canalicular probing, and repair of medial eyelid lacerations where missed canalicular injury leads to chronic tearing. The male-specific morphology can also matter in oculoplastic planning, since medial canthal contour and caruncular exposure influence scar placement and postoperative symmetry.
