Description
From a lateral perspective into the left ventricular cavity, the anterolateral papillary muscle rises from the ventricular free wall and projects centrally toward the mitral valve plane, its apex giving origin to chordae tendineae that course superiorly to the anterior and posterior mitral leaflets. Trabeculae carneae surround the papillary base and merge with the compact myocardium of the left ventricle, while the interventricular septum lies medial to the chamber and the ventricular apex sits inferiorly. The cut edge of the ventricular wall suggests a sectioned heart preparation, exposing the muscular architecture rather than the epicardial surface. Papillary muscle geometry matters because leaflet competence depends on coordinated contraction that maintains chordal tension during systole; disruption at this level produces acute, often dramatic mitral regurgitation. Ischemia from circumflex or diagonal branch disease can weaken the anterolateral papillary muscle, but complete rupture more often involves the posteromedial papillary muscle after an inferior myocardial infarction, a comparison this lateral view invites in teaching and case discussion.
