Description
Palmar skin creases frame the male hand digits in anterior (palmar) view, with digit I (pollex) positioned laterally and digit V medially in standard anatomical position. Beneath the soft tissues, the phalanges are organized as proximal, middle, and distal segments for digits II to V, while the thumb is composed of proximal and distal phalanges only. Interphalangeal joints align distally, and the metacarpophalangeal joints form the proximal hinge line, where the flexor tendon sheaths and pulley system track along the volar surfaces. This palmar presentation matters because most clinically relevant motion loss and pain in the fingers localize to the flexor surface, where tendon excursion depends on intact annular pulleys (A2 and A4 are the usual teaching landmarks) and a smooth synovial sheath. Zone II flexor tendon injuries, the classic “no man’s land,” sit precisely where the sheath is tightest and where postoperative adhesions most often limit proximal interphalangeal joint flexion.
