Description
Posterior to the gastric body (corpus ventriculi), the smooth serosal surface faces the lesser sac (omental bursa) and lies anterior to the pancreas and left kidney, with the lesser curvature positioned medially and the greater curvature sweeping laterally and inferiorly. Superiorly the corpus approaches the fundus beneath the left hemidiaphragm, while inferiorly it narrows toward the antrum and pyloric region. Along the lesser curvature, the peritoneal reflection into the lesser omentum marks the boundary with the epiploic (omental) bursa, and along the greater curvature the attachment for the greater omentum is implied. Orientation from the dorsal side is the one you want when teaching the anatomy of the lesser sac and the posterior gastric wall. This is the surface approached when the gastrocolic ligament is opened to enter the omental bursa during distal pancreatectomy or drainage of a pancreatic pseudocyst, and it is also the wall that can ulcerate into adjacent structures, classically eroding toward the pancreas and splenic artery with posterior gastric ulcer disease. It also helps clarify why posterior wall lesions can present differently on endoscopy and imaging than anterior ulcers that tend to perforate freely into the greater peritoneal cavity.
