Description
Posterior surface anatomy of the male sacrococcygeal region is centered on the coccygeal foveola, a small midline dimple overlying the distal sacrum and coccyx. The indentation sits superior to the intergluteal (natal) cleft and is bounded laterally by the medial contours of the gluteal region. Superiorly, the dimple relates to the sacral apex, while inferiorly it approaches the upper margin of the gluteal cleft as the skin transitions toward the perineal midline. Clinical attention to the coccygeal foveola hinges on what a “simple dimple” is not. A midline pit that sits higher than expected, appears deep, or is associated with drainage, local erythema, or a palpable subcutaneous tract raises concern for a congenital dermal sinus and potential connection toward the sacrococcygeal canal, which can carry a risk of meningitis if infected. Location also helps separate entities: pilonidal disease more often involves the superior intergluteal cleft with secondary pits and hair nests, while a true coccygeal foveola is a focal depression over the coccyx without the inflammatory changes typical of pilonidal sinus.
