Description
Posterior anatomy of the adult male large intestine is rendered with emphasis on the secondarily retroperitoneal segments of the colon. The ascending colon lies on the right, lateral to the right kidney and quadratus lumborum, while the descending colon runs on the left, lateral to the left kidney, both fixed posteriorly where the fusion fascia (fascia of Toldt) tethers them to the posterior abdominal wall. Superiorly, each segment approaches its respective colic flexure, and inferiorly the descending colon continues toward the sigmoid colon in the left iliac fossa. Mesenteric mobility is minimized in these portions by their posterior peritoneal reflection. For operative and radiologic anatomy, this posterior perspective clarifies why the right and left colon behave differently from the transverse and sigmoid colon during mobilization. During right hemicolectomy or left colectomy, safe entry into the lateral peritoneal attachments along the white line of Toldt permits medial mobilization while protecting retroperitoneal structures, including the ureter and gonadal vessels that course on the psoas major. Expect adhesions here. This relationship also explains posteriorly directed tumor invasion into Gerota fascia or the posterior abdominal wall, an issue that drives staging and en bloc resection planning.
