Dorsal (posterior) hand anatomy is presented across the metacarpus, from the bases of metacarpals I to V proximally toward the metacarpophalangeal joints distally, with the second and third metacarpals forming a prominent central ridge and the fourth and fifth rays tapering ulnarly. Subcutaneous extensor tendons course longitudinally over the metacarpal shafts, with the extensor digitorum tendons aligning to digits II to V and extensor indicis and extensor digiti minimi reinforcing the index and little finger on their ulnar and radial sides, respectively. Laterally, the first metacarpal sits oblique and mobile at the thumb carpometacarpal complex, while the ulnar border tracks toward the fifth metacarpal head. Surface contour matters. This posterior perspective is the workhorse view for teaching dorsal compartments and tendon topography, because the extensor mechanism becomes palpable and clinically testable over the metacarpals during finger extension and resisted extension. It also anchors common injury patterns in the mid-hand, including metacarpal shaft fractures (classically the fifth metacarpal neck, Boxer’s fracture) and carpometacarpal fracture-dislocations, where dorsal swelling and loss of knuckle prominence can be the first external clue. The dorsal venous network and thin soft tissue envelope over the metacarpus also explain why lacerations and bite injuries so readily threaten extensor tendons and dorsal capsular structures.
