Description
Posterior to the left lower lobe, the medial basal segmental bronchus (B7) is traced as it branches from the left lower lobar bronchus and courses inferomedially toward the medial basal segment (S7). Just lateral and slightly posterior, the posterior basal segmental bronchus (B10) occupies the posterior basal segment (S10), with the two segmental airways running in close proximity but separating as they pass distally toward their respective segmental parenchyma. Inferior positioning reflects the basal distribution, while the medial course of B7 distinguishes it from the more posterolateral trajectory of B10. Small caliber, big consequence. Posterior segmental anatomy in the left lower lobe matters when you need to think in segments, not lobes, for bronchoscopy planning, CT interpretation, or segmentectomy. The medial basal segment is variable on the left and its bronchus can be diminutive or share branching patterns, so a rear view that emphasizes B7 in relation to B10 helps prevent mislabeling of a lesion’s drainage pathway or the target segment for sampling. This relationship also underpins endobronchial navigation when approaching posteriorly located nodules near the basilar segments, where an off-angle turn into B10 can masquerade as a successful entry into B7. Pulmonary and thoracic anatomy courses can use this piece to teach left lower lobe segmental nomenclature and common variants, while radiology and pulmonology publications will find it useful for figure panels explaining basilar segment orientation on axial CT or during bronchoscopy.
