Posteriorly, the shoulder contour is dominated by the deltoid muscle, with its spinal (posterior) fibers sweeping inferolaterally from the acromion and spine of the scapula toward the deltoid tuberosity on the lateral humeral shaft. Medial to the deltoid cap, the scapular spine runs transversely to the acromion, separating the supraspinous and infraspinous fossae, while the medial border of the scapula tracks vertically along the posterior thoracic wall. The posterolateral edge of the acromion sits superior and lateral, forming the roof of the glenohumeral region. Clear surface landmarks. This posterior survey matters because the deltoid region is a frequent site for intramuscular injection and postoperative portal placement, and a small shift in landmarking changes what you risk.
