Description
Posterior to the pelvis, the intergluteal (natal) cleft runs in the midline between the right and left gluteal regions, extending inferiorly toward the anal verge and superiorly toward the sacrococcygeal area. The skin over the sacrum and coccyx lies superior to the cleft, while the paired buttocks, formed primarily by the gluteus maximus muscle mass deep to the subcutaneous tissue, sit lateral to it. Inferiorly, the perianal skin encircles the anus, continuous with the medial gluteal skin folds. Midline anatomy matters here. This posterior surface view is a practical teaching reference for the anatomy of the rima ani and adjacent cluneal (buttock) region, where dermatologic landmarks guide clinical examination and procedures. The superior natal cleft and sacrococcygeal midline are the typical location for pilonidal disease, including pilonidal sinus and abscess, which often present with pain, drainage, and midline pits just cephalad to the cleft. It also supports orientation for posterior perianal conditions such as fissures, perianal dermatitis, and external hemorrhoids, where documenting location relative to the anal verge and midline affects management and follow-up.
