Posteriorly, the male thigh is dominated by the hamstring compartment, with the biceps femoris laterally and the semitendinosus and semimembranosus forming the medial contour as their muscle bellies descend toward the knee. Superiorly, the posterior thigh blends into the gluteal fold, while inferiorly the muscular mass narrows to frame the popliteal fossa. The popliteal fossa sits centrally between the hamstring tendons above and the heads of gastrocnemius below, a shallow diamond-shaped depression on the back of the knee. Medially, the posteromedial thigh approaches the adductor compartment, while the lateral margin tracks toward the iliotibial tract. Posterior thigh anatomy matters because the hamstrings cross both the hip and knee, so strain patterns and clinical testing depend on where pain localizes along the muscle belly versus the proximal tendon.
