Styloid process of the third metacarpal projects dorsally from the base of the central metacarpal, forming a pointed bony ridge on the posterior aspect of the hand near the carpometacarpal region. From this fixed posterior perspective, the dorsal base of metacarpal III is emphasized relative to the shaft distally, with the styloid positioned proximal and slightly radial compared with the midline of the metacarpal. Adjacent dorsal contours of the second and fourth metacarpal bases are typically implied laterally and medially, helping orient the viewer to the central ray of the hand and its relationship to the capitate at the third carpometacarpal (CMC) joint. A sharp landmark. Clinically, the dorsal styloid area of metacarpal III is best known as the insertion site for extensor carpi radialis brevis (ECRB), so bony prominence or tenderness here often enters the differential for dorsal wrist pain in racquet sports, golfers, and manual workers. Hypertrophy at the dorsal base of the second and third metacarpals, often termed a carpal boss, can irritate overlying extensor tendons and may coexist with degenerative change at the second or third CMC joints.
