Description
Seen from the left side, the maternal torso is rendered semi-transparent so that the gravid uterus dominates the lower abdomen and pelvis. Inside the amniotic cavity a fetus of about 20 weeks lies in a cephalic, curled attitude, its large head directed inferiorly toward the pelvic inlet, limbs flexed against the trunk, and the coiled umbilical cord running from the mid-abdomen to the placental region on the uterine wall. Behind and below the uterus the urinary bladder sits anteriorly against the pubic bone while the rectum passes posteriorly in front of the sacrum. Loops of small intestine are displaced superiorly and posteriorly above the uterine fundus, and the lumbar vertebrae, sacrum and pelvic bones are visible as pale landmarks defining the maternal contour. At mid-gestation the fundus reaches approximately the level of the umbilicus, which makes this the classic view for teaching symphysis-fundal height measurement and the anatomical basis of the 20-week anomaly scan. The lateral projection shows how the enlarging uterus compresses the bladder and displaces the bowel, explaining urinary frequency, reflux and postural lumbar lordosis with associated low back pain, mirrored here by the woman's hand supporting her flank. It also clarifies the spatial relationships used when assessing fetal lie and presentation, and when interpreting sagittal obstetric ultrasound planes.
