Description
Anteriorly, the gallbladder (vesica biliaris) is presented with emphasis on its serosal covering, the visceral peritoneum forming the tunica serosa over the fundus and body. Superiorly and medially, this peritoneal layer reflects toward the inferior surface of the liver, where the gallbladder fossa creates a nonperitoneal attachment bed while the free surface remains smoothly invested. The transition from the broader fundus to the narrower neck is evident inferiorly, with the serosa thinning into peritoneal reflections as the organ approaches the hepatocystic junction. Attention to the serosa matters because it defines surgical planes and the routes by which inflammation spreads in acute cholecystitis. Edema, fibrin, and adhesions form on the peritoneal covering and can tether the gallbladder to the omentum, transverse colon, or duodenum, changing the dissection feel during a laparoscopic cholecystectomy and raising the risk of serosal tears and bile spillage. This anterior perspective also supports teaching of why the gallbladder’s hepatic surface behaves differently, it lacks a full serosal investment where it contacts liver parenchyma.
