Anteriorly oriented, the right atrial appendage (auricula dextra) projects from the anterosuperior aspect of the atrium dextrum, its crenulated free margin forming a serrated contour over the cardiac silhouette. A broad-based junction with the right atrium is implied at the appendage root, with the appendage lying to the right of the interatrial septal plane and superior to the tricuspid orifice region. External pectinate ridging is suggested by the lobulated surface, even though the specimen is presented without the great vessels. No vascular distractions. Clinical relevance centers on the appendage as the most anterior right-sided atrial structure encountered when opening the pericardium and inspecting right atrial anatomy in the operating room. The auricula dextra helps you orient the right atrium relative to the atrioventricular groove, and its pectinate architecture contrasts with the smoother posterior right atrial wall derived from the sinus venarum. Electrophysiology teams also recognize the appendage as a potential site of ectopic atrial activity and as a landmark during mapping near the cavoatrial junction, where catheter position can be confused by the appendage’s trabeculated contour.
