Description
Posteriorly oriented anatomy of the male duodenum focuses on the retroperitoneal second, third, and usually fourth parts, which lie fixed against the posterior abdominal wall while the first part remains comparatively mobile and more intraperitoneal. The descending (second) part sits to the right of the midline, lateral to the head of the pancreas, while the horizontal (third) part crosses the midline anterior to the aorta and inferior vena cava and inferior to the pancreatic head and uncinate process. Superiorly, the duodenojejunal flexure approaches the left side near the ligament of Treitz, with the fourth part ascending to meet proximal jejunum. A tethered gut segment. This posterior perspective matters because retroperitoneal fixation governs both spread and access: a posterior duodenal ulcer can penetrate into the pancreas, while inflammation, hematoma, or perforation may track along the anterior pararenal space rather than freely within the peritoneal cavity. The third part’s relationship to the superior mesenteric artery and aorta underpins superior mesenteric artery syndrome, where compression produces postprandial obstruction, and it also explains why mobilization (Kocher maneuver) is used to expose the inferior vena cava, aorta, and pancreatic head during trauma and pancreaticoduodenal surgery.
