Description
Prominent periorbital edema surrounds both orbits, with a violaceous heliotrope discoloration involving the upper and lower eyelids and extending onto the malar eminences of the zygomatic region. The swelling sits superficial to the orbicularis oculi and tracks laterally toward the temporal fossa, while the midface, nasal bridge, and perioral tissues appear comparatively spared, helping localize the process to the periocular dermis and subcutis. Head-and-shoulders framing keeps attention on the facial cutaneous findings in an adult male phenotype. Heterogeneous periorbital violaceous change is one of the few rashes that can point you toward dermatomyositis before proximal muscle weakness is even volunteered, and this rendering emphasizes that pattern over more common mimics such as allergic contact dermatitis or bruising from trauma.
