Description
Framed on the anterior male neck, the muscular triangle (trigonum musculare, omotracheal triangle) sits anteromedially, bounded by the cervical midline medially, the anterior border of the sternocleidomastoid laterally, and the superior belly of the omohyoid inferolaterally. Within this triangle, the infrahyoid strap muscles occupy the superficial plane, with sternohyoid and sternothyroid running vertically toward the hyoid and thyroid cartilage, and omohyoid coursing obliquely toward its intermediate tendon. Deep to the straps, the laryngeal framework and upper trachea lie in the midline, with the thyroid gland positioned anterolateral to the trachea and inferior to the thyroid cartilage. Surgeons and proceduralists return to this triangle because it is the surface map to the airway and thyroid. Midline access between the strap muscles is the standard route for tracheostomy and cricothyrotomy-related teaching, and the relationship of the thyroid isthmus to the second and third tracheal rings helps explain why the isthmus may need to be divided or retracted during open tracheostomy. This view also supports thyroidectomy orientation by reinforcing how the sternohyoid and sternothyroid must be separated and retracted to expose the gland while protecting adjacent neurovascular structures that lie more laterally.
