Description

Running along the lateral thoracic wall, the serratus anterior occupies the superficial plane, its digitations interleaving with slips of the external intercostal muscles across successive intercostal spaces. Superiorly the region approaches the axillary fossa, while inferiorly it tapers toward the costal margin and lateral abdominal wall. Ribs and their intercostal intervals set the underlying framework, with the intercostal tissues spanning between adjacent costae from posterior toward anterior. Orientation is strictly lateral. Clinically, this is the surface anatomy corridor used to teach safe access to the pleural cavity and to anticipate where instruments traverse intercostal layers before reaching endothoracic fascia and pleura. The relationship between the serratus anterior and the intercostal spaces also frames common pain patterns in intercostal neuralgia and guides rib fracture assessment, while reminding the operator that the intercostal neurovascular bundle tracks along the inferior border of each rib and is avoided by keeping thoracostomy and drains just superior to the rib below.
