Description
Rendered in a lateral profile of the female pelvis, the cervix uteri and upper vagina are shown distorted by an invasive cervical carcinoma with gross extension beyond the cervical stroma into the paracervical tissues (parametrium). Tumor spread tracks laterally toward the pelvic sidewall along the cardinal (transverse cervical) ligament region, bringing the mass into close relationship with the distal ureter as it courses inferiorly to the bladder. Anteriorly the urinary bladder and vesicouterine space are displaced, while posteriorly the rectum and rectouterine pouch lie immediately behind the cervix. Stage III disease matters because it is defined by local extent that changes both symptoms and management: involvement of the lower third of the vagina (FIGO IIIA), extension to the pelvic wall or encasement of the ureter with obstructive uropathy (FIGO IIIB), and/or nodal disease (IIIC) all move the patient out of primary surgical pathways and into definitive chemoradiation planning. Ureteral compression at the pelvic brim or along the parametrial tunnel can lead to hydroureter and hydronephrosis, a common driver of renal insufficiency that must be recognized on imaging and addressed with stenting or nephrostomy before or during treatment. Gross parametrial involvement is also the anatomic correlate of a fixed cervix on bimanual exam. It predicts poor operability.
