Description
Medial pterygoid plate anatomy anchors this illustration of the sphenoid’s pterygoid process, emphasizing the slender medial lamina descending inferiorly from the body of the sphenoid. Along its inferior end, the plate terminates in the pterygoid hamulus, a hook-like projection positioned posteroinferiorly and slightly lateral to the posterior margin of the hard palate. Medial to the plate lies the nasopharyngeal space and posterior nasal aperture (choana), while the lateral pterygoid plate sits lateral and slightly posterior, together framing the pterygoid fossa between them. Fine bony contours at the plate’s posterior border and at the hamular tip are rendered with enough relief to appreciate the change from broad superior attachment to narrow inferior termination. Clinically, the hamulus is a small landmark with outsized relevance: it functions as a trochlea for the tendon of tensor veli palatini as the tendon turns around the hamulus to enter the palatine aponeurosis, and irritation at this interface is implicated in hamular bursitis and referred otalgia. Fracture or iatrogenic elongation of the hamulus can contribute to palatal pain, dysphagia, and Eustachian tube dysfunction, so having the bony hook and its orientation in mind matters when correlating symptoms to the skull base. The fixed perspective makes the plate’s inferior taper and the hamulus’ medial hook geometry easy to judge. Small structures matter.
