Description

Hierarchical skeletal muscle organization is presented from whole muscle to ultrastructure, with epimysium investing the muscle belly and perimysium partitioning it into fascicles that contain individual muscle fibers. Each fiber is bounded by sarcolemma and filled with sarcoplasm, and the cutaway magnification resolves tightly packed myofibrils arranged in parallel along the long axis. At that scale, the repeating A bands and I bands define the striated pattern, with sarcomeres aligned in register across adjacent myofibrils and the sarcoplasmic reticulum wrapping each myofibril as a sleeve-like network. Small arteries and veins course within the connective tissue septa, and a peripheral nerve approaches the fascicles to supply the fibers. This layered view matters because most clinically relevant muscle problems localize to a specific level, and teaching that distinction is easier when the anatomy is shown as a continuous hierarchy. Disuse atrophy and many myopathies reduce fiber caliber, but the contractile deficit tracks with disruption of sarcomere organization, including loss of normal A and I band regularity on histology. The connective tissue compartments are surgical landmarks as well, since intramuscular bleeding or infection tends to track along perimysial planes and neurovascular branches travel with those septa.
