Centered on the anterior shoulder and upper chest, the deltopectoral (clavipectoral) triangle is defined by the lateral border of pectoralis major medially, the anterior margin of deltoid laterally, and the inferior surface of the clavicle superiorly. Between deltoid and pectoralis major, the deltopectoral groove creates a shallow infraclavicular fossa, with the coracoid process palpated deep and slightly inferior to the clavicle as a key bony landmark. Superficial contours also reflect the female breast and the transition from the clavicular head of pectoralis major to the deltoid cap over the proximal humerus. Surface anatomy matters here. Clinicians rely on this triangle because it is the standard window for the deltopectoral approach to the shoulder, used in open reduction and internal fixation of proximal humerus fractures and in shoulder arthroplasty, where the surgeon develops the interval between deltoid and pectoralis major to protect the deltoid while gaining access to the glenohumeral region. The cephalic vein commonly runs in this groove and pierces the clavipectoral fascia to join the axillary vein; losing it during dissection can complicate exposure and obscure the course of the thoracoacromial vessels. Teaching the palpable clavicle, coracoid, and deltopectoral interval together reduces errors in landmarking for infraclavicular injections and for planning incisions that avoid tethering scars across the shoulder.
