Description

Viewed from a superior (cranial) perspective, the pancreatic head (caput pancreatis) occupies the right upper abdomen and sits cradled within the C-shaped curve of the duodenum. The duodenal second part lies lateral to the head, with the third part coursing inferiorly and posteriorly, framing the uncinate process that projects medially. Posterior to the pancreatic head, the expected groove for the common bile duct aligns toward the posterolateral surface, while the pancreatic neck trends medially toward the body at the level where the portal venous confluence typically forms. Short axis relationships read clearly from above. Surgeons and endoscopists care about this top-down geometry because the duodeno-pancreatic interface dictates safe corridors during pancreaticoduodenectomy (Whipple) and during kocherization of the duodenum, when mobilization exposes the posterior head, the inferior vena cava, and the pancreaticoduodenal vascular arcades. That relationship also explains why a pancreatic head carcinoma commonly presents with painless obstructive jaundice, the intrapancreatic segment of the common bile duct is vulnerable where it runs in a posterior groove before entering the second part of the duodenum at the major duodenal papilla. Orientation from the cranial side makes the “wrap” of the duodenum around the head intuitive. A practical landmark.
