The fibular (peroneal) tendons, longus and brevis, course inferiorly and anteriorly toward the lateral midfoot, while the sural nerve and small saphenous vein run superficially in the posterolateral ankle and can be appreciated by their typical path and surface contour. Superiorly, the distal fibula defines the bony prominence, and inferiorly the calcaneal tuberosity and lateral heel pad shape the plantar transition. Bony landmark, soft tissue gutter, and tendon trajectory align in one profile. Palpation of this region guides both diagnosis and procedure. Peroneal tendinopathy and tenosynovitis commonly localize just posterior to the lateral malleolus, and tendon subluxation often relates to injury of the superior fibular retinaculum after an inversion sprain, a mechanism that also stresses the anterior talofibular and calcaneofibular ligaments nearby.
