Description
Anterior abdominal viscera are arranged within the female torso with the liver occupying the right upper quadrant, its inferior margin draping over the gastric antrum and proximal duodenum. Medial and slightly left, the stomach lies superior to the transverse colon, while the pancreas tracks retroperitoneally across the posterior abdominal wall from the duodenal C loop toward the splenic hilum. Centrally, jejunal and ileal loops fill the infracolic compartment, framed peripherally by the haustrated large intestine, ascending on the right, transverse across the upper abdomen, and descending on the left into the sigmoid colon. Reflections of peritoneum define the greater and lesser sacs and tether the viscera via mesentery and omenta. Orientation in this projection matters when teaching peritoneal compartments and predicting patterns of pain and fluid spread. Foregut pathology such as peptic ulcer disease or pancreatitis often refers pain to the epigastrium and back because the pancreas sits posterior to the stomach and is secondarily retroperitoneal, while colicky pain from small-bowel obstruction concentrates around the periumbilical region as distended loops crowd the midline. Small details guide real decisions: the relationship of the transverse colon and mesocolon to the stomach and pancreas is the map for entering the lesser sac during distal gastrectomy, and it explains why inflammation can track along the paracolic gutters into the pelvis. Landmarks, not guesswork.
