Description
Occipital plane (planum occipitale) occupies the smooth, convex superior portion of the squama occipitalis, positioned posteriorly on the cranium above the superior nuchal line. From a posterior external skull perspective, the plane sits inferior to the lambdoid suture where the occipital bone meets the parietal bones, and it extends medially toward the external occipital protuberance (inion) and laterally toward the posterolateral corners of the squama. Inferior to this plane, the superior nuchal line marks the transition to the rougher nuchal surface where ligamentous and muscular attachments become dominant. Smooth cortical contour dominates. For surface anatomy and surgical planning, the occipital plane helps you separate the scalp bearing attachments of the occipitalis and epicranial aponeurosis superiorly from the nuchal musculature inferiorly, including trapezius and sternocleidomastoid attachments that cluster around the superior nuchal line and external occipital protuberance. In blunt head trauma, occipital fractures often propagate toward the lambdoid suture or down toward the foramen magnum, so recognizing the plane above the superior nuchal line helps orient fracture lines and potential posterior fossa risk on CT. The fixed lighting and smooth bony texture make the change in surface character at the nuchal line easy to teach.
