Curving along the posterior border of the os coxae, the greater sciatic notch is traced as the ilium transitions inferiorly into the ischium, just posterior to the acetabulum and lateral to the pelvic cavity. The animation orients the viewer to the bony margins by sweeping from the posterior superior iliac spine down to the posterior inferior iliac spine, then into the concavity of the notch and toward the ischial spine and lesser sciatic notch. Relative position is made explicit: the notch sits posterior and inferior to the iliac fossa, posterior to the hip joint, and superior to the ischial tuberosity. Rotational cues clarify the notch’s relationship to the pelvic brim and to the lateral surface of the pelvis (pelvic girdle). Clinically, this is not just a surface feature of the hip bone. The greater sciatic notch becomes the greater sciatic foramen when bridged by the sacrospinous ligament, creating the principal exit from the pelvis for the piriformis muscle and the sciatic nerve, along with the superior and inferior gluteal vessels and nerves and the pudendal neurovascular bundle (which hooks around the ischial spine). Seeing the notch’s margins in sequence helps learners understand why piriformis syndrome, deep gluteal syndrome, and posterior hip approaches must respect the bony-ligamentous gateway formed between the ilium and ischium. Landmarks matter.
