Layered from superior to inferior in a posterior dissection-style view, the paired lungs occupy the thoracic cavities lateral to the mediastinum, with the heart sitting anterior to the esophagus and thoracic vertebral column and only partly appreciated from behind. Inferior to the diaphragmatic domes, the liver lies predominantly in the right upper quadrant with its posterior surface approaching the right hemidiaphragm, while the spleen sits more superior and lateral on the left, tucked under the posterior ribs. Retroperitoneally, the kidneys lie on either side of the vertebral column with the right kidney slightly more inferior, capped by the suprarenal (adrenal) glands, and the pancreas is positioned deep and anterior to these structures behind the stomach. Loops of jejunum and ileum fill the central abdomen, and the large intestine frames them, with the ascending and descending colon running more laterally and the sigmoid colon tending toward the left lower abdomen. Posterior relationships matter when you are trying to explain why renal pain refers to the costovertebral angle and why retroperitoneal infection or hemorrhage can spread without early peritoneal signs. This orientation also aligns with common operative corridors, including posterior retroperitoneal access for nephrectomy or adrenalectomy and posterolateral thoracotomy where the lung bases and diaphragm define the inferior limit of exposure.
