Description
Positioned in the right upper quadrant, the liver occupies the hypochondriac and epigastric regions, tucked immediately inferior to the right hemidiaphragm and deep to the costal margin. Its convex diaphragmatic surface faces superiorly and anteriorly, while the inferior visceral surface relates to the gastric antrum and first part of the duodenum medially and the hepatic flexure of the colon inferolaterally. The right lobe dominates laterally, with the left lobe extending across the midline toward the left upper abdomen. Subtle surface contouring suggests the falciform ligament plane separating right and left anatomical lobes on the anterior aspect. Localization matters in practice. Palpation and percussion of the liver edge along the right midclavicular line, as well as bedside recognition of hepatomegaly, depends on understanding how the inferior margin normally sits just beneath the ribs in quiet respiration and descends with inspiration. This anterior body-context view also supports interpretation of right upper quadrant pain patterns seen in acute hepatitis or biliary disease, and it helps explain why subdiaphragmatic irritation from hepatic capsular distension can refer pain to the right shoulder via the phrenic nerve. A practical surface landmark. Use this figure in gross anatomy and clinical skills teaching to anchor liver anatomy to surface anatomy, or in patient-facing hepatology and general surgery materials when explaining where the hepatic gland sits relative to ribs, diaphragm, stomach, and proximal duodenum. It also suits radiology and cross-sectional anatomy introductions as a prelude to ultrasound windows and CT orientation of the upper abdomen.
