Running longitudinally along the posterior thorax, the semispinalis thoracis appears as a deep, intrinsic back muscle of the transversospinalis group, positioned medial to the costal angles and immediately posterior to the thoracic transverse processes. Its fibers arise from transverse processes of the lower thoracic vertebrae (classically T6 to T10) and pass superomedially to insert onto the spinous processes of upper thoracic and lower cervical vertebrae (approximately C6 to T4), crossing multiple motion segments. Superficial layers such as trapezius and rhomboid major overlie this compartment, while the semispinalis thoracis blends cranially with semispinalis cervicis and lies adjacent to multifidus in the paraspinal gutter. Deep muscle, long lever arm. Clinically, this view matters when teaching segmental extension and contralateral rotation of the thoracic spine, because semispinalis thoracis spans several vertebrae and contributes to coupled motion and postural endurance rather than single-level stabilization. Paraspinal pain generators often sit in the same neighborhood: dorsal rami and medial branches traverse the posterior elements near the transverse processes and facet joints, and surgical approaches that strip the posterior musculature (laminotomy, laminectomy, instrumentation) can denervate or atrophy the transversospinalis mass. Palpation and dry needling targets are frequently misattributed to more superficial scapular retractors, so a deep-layer orientation reduces common teaching errors.
