Along the midline, the spinous processes align from C7 to the sacrum, flanked bilaterally by the ribs as they curve laterally from the thoracic vertebrae toward the costal angles. Superolaterally, each scapula sits over ribs 2 to 7 with its spine and acromion directed laterally toward the proximal humerus, while inferiorly the paired os coxae frame the sacrum at the posterior pelvis and continue into the proximal femora at the hip joints. This posterior view matters when teaching surface anatomy and bony landmarks that guide examination and procedures, because the scapular spine, inferior angle, iliac crests, and sacral hiatus map directly to clinically used levels and access points. It also supports interpretation of posterior imaging and deformity patterns: thoracic scoliosis, kyphosis, vertebral compression fractures, and scapular winging (often related to long thoracic nerve dysfunction affecting serratus anterior) are best understood when the rib cage and scapulothoracic relationship are seen together.
