Description
Uterus didelphys appears as paired uterine bodies positioned side by side within the pelvis, separated by a deep midline cleft that reflects complete nonfusion of the paramesonephric (Müllerian) ducts. Each hemiuterus contains its own endometrial cavity, with separate myometrial walls and a distinct fundus oriented superiorly. Inferiorly, two cervices descend toward the upper vagina, typically lying parallel with a narrow band of intervening tissue, while the adnexal regions (uterine tubes and ovaries) would be expected laterally if included in frame. Two cervices are present. This configuration matters because it reframes “uterine anomaly” from a single distorted cavity to two independent reproductive tracts, which directly affects obstetric counseling and procedural planning. Uterus didelphys is associated with malpresentation, preterm birth, and recurrent pregnancy loss, and it can complicate cervical assessment because each cervix may dilate or efface differently; the fixed view that keeps both uterine cavities and cervices in the same plane helps clarify why imaging reports must specify laterality and which cavity contains a gestation.
