Description
Rendered in anterior orientation, the sectioned male kidney exposes the renal cortex peripherally and the medulla centrally, with renal pyramids converging toward the papillae and minor calyces before draining into major calyces and the renal pelvis. A lobulated malignant mass expands within the renal parenchyma, distorting the corticomedullary junction and compressing adjacent pyramids and collecting system spaces. Inferiorly, the renal pelvis funnels into the proximal ureter at the ureteropelvic junction. Normal nephron location is implied, with glomeruli concentrated in the cortex and loops of Henle extending into the medulla. This cut plane is the one clinicians mentally reference when correlating imaging with gross pathology in suspected renal cell carcinoma, where a solid parenchymal mass can displace calyces, invade the renal sinus, and obstruct urinary outflow. Mass effect on the collecting system helps explain hematuria and flank pain, while proximity to the renal pelvis and ureter frames the mechanism for hydronephrosis when the ureteropelvic junction becomes narrowed. It also sets up a discussion of staging, since extension beyond the renal capsule or into perihilar structures changes surgical planning.
