Description

Posterior to the vertebral bodies and intervertebral discs, the ligamentum flavum spans between adjacent laminae along the cervical and upper thoracic spine in a lateral (sagittal) skeletal view. Its elastic bands sit deep to the spinous processes, forming part of the posterior wall of the vertebral canal, while the ribs articulate with the thoracic vertebrae just lateral to the zygapophyseal joints. Superiorly, the occiput and mandible frame the craniocervical junction, and inferiorly the thoracic cage anchors the segmental rhythm of the upper trunk. Clear bony landmarks surround it. Functionally, the flavum ligament matters because its high elastin content maintains resting tension across the interlaminar space and supports recoil during extension, helping preserve canal geometry between laminae. Hypertrophy, buckling, or ossification of the ligamentum flavum contributes to cervical or thoracic spinal stenosis, a common culprit in myelopathy when the posterior canal encroaches on the cord during extension or with degenerative facet loading.
