Description
Posterior thoracic back anatomy is centered on the iliocostalis thoracis, the lateral column of the erector spinae, running longitudinally just lateral to the thoracic spinous processes and deep to the more superficial scapular musculature. Its fascicles course superiorly toward the angles of the ribs, forming a muscular band parallel to the vertebral column, while the longissimus thoracis sits immediately medial to it. Superficial landmarks of the upper back and shoulders frame the field, including the scapulae and deltoid contours, with the rhomboid major and rhomboid minor highlighted between the medial border of the scapula and the thoracic vertebrae. Bony reference points include the thoracic vertebral column in the midline and the scapular spines laterally. Clinically, this view clarifies why thoracic paraspinal pain can localize lateral to the spinous processes: iliocostalis thoracis strain and myofascial trigger points often refer discomfort along the posterolateral rib cage, and rib angle tenderness can mimic costovertebral joint irritation. A common palpation pitfall is confusing rhomboid fibers with the deeper erector spinae columns. Depth matters.
