Description

Posterior surface anatomy of the knee is centered on the popliteal fossa, a diamond-shaped depression bounded superolaterally by the biceps femoris and superomedially by the semimembranosus and semitendinosus tendons. Inferiorly, the medial and lateral heads of the gastrocnemius converge toward the calcaneal (Achilles) tendon, framing the proximal calf as the femur transitions into the tibia across the posterior joint line. A circumferential highlight over each knee draws attention to the posterior condylar region where the distal femur sits superior to the tibial plateau, with the fibular head palpable laterally just distal to the joint. Clinically, this posterior view matters because the popliteal fossa is a crowded corridor where the popliteal artery and vein lie deep against the posterior femur and capsule, while the tibial nerve runs superficially and centrally and the common fibular (peroneal) nerve tracks laterally toward the fibular neck. Small changes in volume here are diagnostic: a Baker (popliteal) cyst typically presents as a posteromedial fullness between the semimembranosus and medial gastrocnemius, and a popliteal artery aneurysm can produce a pulsatile mass with distal ischemic symptoms. Palpation and needle trajectories depend on these relationships. It is a tight space.
