Description
Posterior upper back anatomy is presented with the cervical and upper thoracic spinous processes in the midline, ribs laterally, and both scapulae overlying the posterolateral thoracic cage. Deep to the superficial trapezius, the rhomboid minor runs from the nuchal ligament and spinous processes of C7 to T1 to the medial border of the scapula at the level of the scapular spine, while the rhomboid major continues inferiorly from the spinous processes of T2 to T5 to the medial border down toward the inferior angle. Their fibers course inferolaterally from vertebral column to scapula, lying medial to the levator scapulae superiorly and blending along the contiguous insertion on the scapular medial border. Clear attachment geography. This posterior relationship matters when teaching scapulothoracic mechanics and when troubleshooting periscapular pain. Rhomboid weakness or inhibition shifts the scapula into lateral translation and downward rotation deficits, a common contributor to scapular dyskinesis in overhead athletes, while myofascial trigger points in the rhomboids can mimic cervical radiculopathy with medial scapular border pain. The layer concept is also surgical: splitting trapezius to access the dorsal scapular nerve and artery along the medial scapular border puts the rhomboids directly in the field, and the nerve is susceptible to traction or entrapment where it courses deep to levator scapulae.
