Description
Anterior thoracic anatomy is presented with the sternum and adjacent costal cartilages centered, ribs curving laterally toward the costovertebral joints, and the thoracic vertebrae visible posteriorly through the rib cage. Deep to the anterior chest wall, the transversus thoracis (sternocostalis) fans superolaterally from the posterior surface of the inferior sternum and xiphoid region to the internal surfaces of the costal cartilages, typically ribs 2 to 6. Intercostal musculature occupies the intercostal spaces, with fibers oriented obliquely between adjacent ribs and framing the parasternal region where the transversus thoracis lies closest to the pleura. Clavicular and scapular elements appear superiorly and laterally as bony landmarks for the upper thorax. Transversus thoracis matters because it defines the parasternal plane that surgeons and proceduralists traverse when placing anterior chest incisions and parasternal ports, and because it lies immediately superficial to the internal thoracic vessels as they descend along the lateral border of the sternum. Injury to the internal thoracic artery or its anterior intercostal branches is a real risk during sternal wire placement, mediastinal re-entry, or parasternal drainage, and this muscle helps you anticipate that vascular corridor.
