Crus cerebri (basis pedunculi) forms the ventral white matter of the mesencephalon, appearing as paired, longitudinal bundles separated by the interpeduncular fossa along the midline. Medial to each crus, the oculomotor nerve (CN III) rootlets typically emerge from the medial aspect of the cerebral peduncle just inferior to the posterior perforated substance, while the substantia nigra lies immediately dorsal to the crus as a dark lamina demarcating tegmentum from basis. Superiorly the crura converge toward the diencephalon, and inferiorly they continue into the ventral pons, maintaining a medial to lateral organization of descending motor fibers within the cerebral peduncles. White matter dominates the field. Clinically, the crus cerebri matters because it concentrates corticospinal, corticobulbar, and corticopontine tracts in a compact ventral corridor where small lesions produce large motor deficits. Midbrain infarction from paramedian perforators of the posterior cerebral artery, compressive uncal herniation against the cerebral peduncle, or a ventral midbrain tumor can injure these fibers and yield contralateral weakness, with adjacent CN III involvement producing the classic Weber syndrome pattern (ipsilateral oculomotor palsy with contralateral hemiparesis). A ventral emphasis helps anchor neuroanatomy to the bedside: when you localize a pure motor syndrome to the cerebral peduncle, you are localizing to the crus.
