Description
Centered on the anterior scalp and superior cranium, the epicranium is highlighted over the frontal and parietal regions, tracking the contour from the supraorbital area superiorly toward the vertex. Superficial skin and dense connective tissue of the scalp sit outermost, while the galea aponeurotica (epicranial aponeurosis) spans between the frontal belly of occipitofrontalis anteriorly and the occipital belly posteriorly. Laterally, the epicranial layer blends toward the temporal fascia, with the frontal hairline and forehead providing surface landmarks for the underlying frontal bone and paired parietal bones. Orientation is straightforward. Anterior. For teaching, this region is less about the calvaria as bone and more about the scalp as a layered surgical plane: skin, subcutaneous tissue, galea, loose areolar tissue, and pericranium, each behaving differently in trauma and operative exposure. Lacerations that violate the galea gape because frontalis and the epicranial aponeurosis retract, and bleeding can be brisk due to the tethered superficial vessels, while blunt trauma may produce a subgaleal hematoma that spreads widely across the loose areolar plane. The anterior focus also suits discussions of cosmetic and reconstructive anatomy, including frontalis chemodenervation (Botox) for forehead rhytids and placement of coronal or pretrichial incisions that elevate the scalp flap while protecting the supraorbital and supratrochlear neurovascular bundles.
