Description
Interthalamic adhesion (massa intermedia) spans the midline between the medial surfaces of the right and left thalami within the diencephalon. Centered in the third ventricle, this compact bridge appears as a nodular or bandlike connection positioned medial to each thalamus and bordered by ependymal-lined ventricular space superiorly and inferiorly. The adjacent ventricular walls frame its relationship to key midline landmarks such as the hypothalamus inferiorly and the roof of the third ventricle superiorly. Small structure, tricky location. Clinical relevance comes from its variability and its proximity to pathways targeted in neuroendoscopy and ventricular procedures: the interthalamic adhesion may be prominent, thin, duplicated, or absent, and that anatomic variance changes the internal contour of the third ventricle on MRI and during endoscopic navigation. A fixed midline perspective that emphasizes the medial thalamic surfaces helps differentiate the adhesion from true commissural fibers (for example, the anterior commissure) and from the surrounding ventricular boundaries, reducing common learner errors when interpreting sagittal and axial neuroimaging. Size and presence are also discussed in the context of developmental anatomy and midline diencephalic malformations, where third-ventricle configuration and thalamic apposition matter.
