Description
Atlas (C1) anatomy centers on the lateral mass, where the transverse ligament tubercle forms a discrete medial bony prominence along the inner aspect of the lateral mass. Adjacent to this eminence, the medial surface of the lateral mass borders the vertebral foramen and relates to the dens of the axis (C2), while the superior articular facet lies superiorly for the occipital condyle and the inferior articular facet lies inferiorly for C2. The tubercle sits medial to the bulk of the lateral mass and deep to the ring created by the anterior and posterior arches. Small, specific, and easy to miss. Function follows form in the upper cervical spine. The transverse ligament of the atlas attaches to these medial tubercles bilaterally, spanning posterior to the dens to restrain anterior translation of C1 on C2, and disruption at this attachment is a defining mechanism for atlantoaxial instability in high-energy trauma, rheumatoid arthritis, and certain congenital conditions. The fixed viewpoint isolates the tubercle as a landmark on the medial lateral mass, helping the viewer separate true ligament attachment bone from the nearby articular surfaces and the fovea dentis (facet for the dens) on the posterior aspect of the anterior arch.
