Description
Cervical vertebral transverse processes present a paired anterior tubercle and posterior tubercle, with the intertubercular sulcus (sulcus nervi spinalis) running between them as a shallow groove for the ventral ramus of the cervical spinal nerve. Lateral to the pedicle, the transverse process expands from the vertebral arch and, in typical C3 to C6 levels, incorporates the transverse foramen (foramen transversarium) within its root. The anterior tubercle lies anterolateral to the posterior tubercle, while the sulcus sits posterior to the anterior tubercle and immediately anterior to the posterior tubercle in a fixed osseous relationship. Bone surface detail is the point. Surgeons and proceduralists care about these landmarks because they map the safe corridors and the hazards of the lateral neck. The anterior tubercle of C6, the carotid tubercle, is palpated to compress the common carotid artery and serves as a landmark near the cervical sympathetic trunk, where needle misplacement during cervical plexus block or anterior approaches can cause vascular puncture or Horner syndrome. Fractures or osteophytes involving the transverse process can encroach on the intertubercular sulcus and irritate the exiting nerve root, and involvement of the foramen transversarium raises concern for vertebral artery injury in cervical trauma. Muscle attachments also benefit from this bony map, with the scalenus anterior attaching to anterior tubercles and the scalenus medius to posterior tubercles, a relationship that explains how hypertrophy can tighten the interscalene interval.
