Description
Centered on the anterior nasal apex, the lobule and tip are defined by the paired major alar cartilages (greater alar cartilages) shaping the domes and the intervening interdomal region at the midline. Inferiorly, the columella separates the bilateral nares, with the soft tissue triangles located just lateral to the domes at the junction between the tip and the alar lobules. Superior to the tip, the dorsum nasi leads toward the nasal bones, while laterally the alae curve posterolaterally to form the alar rims and the boundaries of the external nasal valve. Small structures, big consequences. This straight-on perspective matters because subtle asymmetry at the lower lateral cartilage domes, the alar margin, or the columellar show is what drives both aesthetic planning and functional complaints after rhinoplasty. Tip support depends on the medial and lateral crura and their fibrous attachments, and weakening or malposition can narrow the external nasal valve, predisposing to inspiratory collapse and persistent obstruction. For teaching, the anterior view is also the cleanest way to discuss how skin thickness and subcutaneous fibrofatty tissue over the tip modify the apparent contour even when the underlying cartilage framework is symmetric.
