Description
Prominent in the anterior brachium, the biceps brachii occupies a superficial position, its long and short heads converging into a distal tendon that courses inferiorly toward the cubital fossa. Deep and slightly medial to the biceps, the brachialis forms the muscular floor against the anterior surface of the humerus, while the coracobrachialis sits proximomedially, running from the coracoid process to the medial humeral shaft. Along the medial aspect, the neurovascular bundle is implied in its usual relationship, with the brachial artery and accompanying veins traveling adjacent to the median nerve, and the musculocutaneous nerve emerging between biceps and brachialis. Key surface anatomy. Anterior compartment anatomy matters because it is where function, examination, and iatrogenic risk intersect: elbow flexion and forearm supination are driven by biceps brachii, while brachialis remains the primary flexor regardless of forearm position. The distal biceps tendon and bicipital aponeurosis are central to understanding distal biceps ruptures and their clinical sign pattern, and they also explain why venipuncture or arterial cannulation in the cubital fossa must respect the brachial artery and median nerve lying deep to the aponeurosis. Surgeons approaching the distal humerus or repairing the distal biceps use these planes and intervals for safe exposure.
