Description
Seen from an anterior position, the kidney presents a smooth anterior renal surface invested by the fibrous capsule, with the superior pole lying closer to the ipsilateral suprarenal gland and the inferior pole tapering toward the ureteropelvic junction. A subtle medial concavity marks the region of the renal hilum, where the renal vein typically lies anterior to the renal artery, and the renal pelvis sits most posterior before continuing inferiorly as the ureter. Laterally, the convex border rounds toward the renal fascia, while the medial border angles toward the great vessels. Anterior surface anatomy matters because it is the surface encountered in most transabdominal sonographic and CT correlations, and it is also the face you orient to when describing the relationships of the right kidney to the liver and second part of the duodenum, or the left kidney to the stomach, spleen, pancreas, and splenic flexure. Orientation at the hilum is a frequent stumbling point in teaching and in operative note writing. It also frames common clinical discussions such as anterior calyceal access planning in percutaneous nephrolithotomy and the risk of bowel injury when traversing an anterior approach corridor.
