Description
Posterior anatomy of the female axial skeleton is presented with the vertebral column centered from the occiput to the upper lumbar region. Superiorly, the occipital bone and posterior cranial vault frame the atlanto-occipital junction, while the cervical vertebrae (C1 to C7) align inferiorly toward the cervicothoracic transition. Laterally, the scapulae sit on the posterior thoracic wall over the ribs, and proximal humeri extend inferolaterally from the glenoid fossae. The thoracic cage encircles the thoracic vertebrae, whose spinous processes form a midline ridge that leads into the lumbar region where the normal lumbar lordosis is suggested by the column’s curvature. Sagittal spinal curves are easier to teach when you can contrast cervical and lumbar lordosis with thoracic kyphosis, even from a posterior projection, because the change in vertebral orientation and rib-bearing thoracic segment remains apparent relative to the scapular plane. Clinical conversations often start here: exaggerated thoracic kyphosis in osteoporotic wedge fractures, loss of cervical lordosis with muscle spasm after whiplash, or compensatory lumbar hyperlordosis associated with spondylolisthesis at L5 to S1. Alignment matters. The posterior midline also provides the landmarks clinicians palpate for neuraxial procedures, with cervical and thoracic spinous process angulation differing in a way that affects needle trajectory and level counting.
