Description
Running along the anterior compartment of the upper arm, the biceps brachii is rendered with its short head arising from the coracoid process and its long head coursing from the supraglenoid tubercle into the intertubercular sulcus before joining the fusiform muscle belly. Distally, the biceps tendon narrows anterior to the elbow joint to insert on the radial tuberosity, with the bicipital aponeurosis spreading medially into the antebrachial fascia. Deep and slightly lateral to the biceps lies the brachialis on the anterior humerus, while the radius and ulna sit inferiorly in the forearm with the hand skeleton aligned distally. Muscle appears in red, tendon in white, and bone in a light neutral tone for clear tissue separation. Orientation is anterior, with osseous landmarks kept readable. Teaching the biceps as a two headed muscle often misses the clinically relevant detail: the long head tendon is intra-articular at the shoulder and commonly inflamed or unstable in the bicipital groove, creating anterior shoulder pain and a snapping sensation with rotation. Distally, the relationship between the biceps tendon, radial tuberosity, and bicipital aponeurosis matters in distal biceps tendon rupture, where loss of supination strength and proximal retraction produces the classic Popeye deformity. This is the anatomy you palpate and test. It is also the corridor surgeons respect during anterior approaches around the elbow and proximal forearm.
