Description
Presented in anterior orientation with the knee in flexion, the distal femur sits superior to the proximal tibia, with the medial and lateral femoral condyles articulating against the corresponding tibial plateaus at the femorotibial joint. Anteriorly, the patella occupies the trochlear (patellar) surface of the femur, centered between the condyles and displaced slightly inferior as flexion deepens. The intercondylar region and tibial intercondylar eminence lie centrally, forming the osseous corridor where the cruciate ligaments course, even if the ligaments themselves are not rendered. Flexion is where joint congruence and contact mechanics change fastest, so this angle is a workhorse for teaching and for clinical reasoning about anterior knee pain. As the patella tracks distally within the trochlear groove, small variations in trochlear depth, patellar height (patella alta or baja), or rotational alignment can concentrate contact pressure and aggravate patellofemoral chondromalacia. An anterior view also maps cleanly to common exam and procedural landmarks, including the patellar facets, the tibial tuberosity region just distal to the joint line, and the corridor used for patellofemoral arthroscopy portals.
